Pilot Scheme on Community Care Service Vouchers for the Elderly
Introduction
“Gracious Alliance Rehabilitation Service” is recognized service provider of the third phase of the Social Welfare Department’s (SWD) “Community Care Service Voucher for the Elderly” pilot scheme. Starting from October 2020, it will be taking home-based and day care center as a basic model to provide Elderly who are under long term nursing with community care services.
Qualification
Under the unified assessment mechanism of the SWD’s elderly services, they are assessed as moderately or severely impaired, and are currently on the central waiting list for subsidized long-term care services, waiting for subsidized community care services and/or residential care services, but have not yet received any Elderly in residential care services or subsidized community care services.
The SWD will directly issue invitation letters to eligible users on the Central Waiting List, inviting them to participate in the third phase of the pilot project. Eligible elders can apply for community voucher after receiving the invitation letter. For assistance in filling out the application form, the elderly can contact the staff in charge or the Central Working Group of the SWD Community Voucher Pilot Scheme.
Service Value (Voucher Value)
The third phase of the pilot project provides five community voucher service portfolio values for the elderly to choose from (prices for 2020-21: HKD 9,870, HKD 8,380, HKD 7,460, HKD 5,970 and HKD 4,130 per month). The elderly can choose the appropriate combination of community voucher service value according to their personal needs. The service portfolio value of the community voucher will be adjusted annually according to the comprehensive consumer price index.
Service Target
- Senior Citizens aged 60 or above living in Southern District; and
- Senior Citizens who have been invited by the Social Welfare Department to participate in the Pilot Scheme on Community Care Service Vouchers for the Elderly

Withdrawing from Service
If you want to withdraw/terminate the service, you must notify the staff of this unit by phone/writing/visiting the office ONE Week in advance.
